3. Make your selection from the dropdown menu to identify the nearest DFA facility. Following this, the address and contact details of the chosen DFA site will become visible. Proceed by clicking “Next” once your preference is confirmed.

In-person presence

On the day of your scheduled DFA Passport Appointment, it’s time to drive to the DFA branch you’ve designated. Make sure to arrive 30 minutes prior to your appointment time. Here’s what comes next:

1. Have all the required documents in hand.
2. Bring along the duly accomplished application form (downloaded from the website or the email).
3. Adhere to the appropriate dress code (smart casual attire with collared shirts for men).
4. Personal appearance is mandatory for all applicants, regardless of age.
5. In the case of minors, a parent or guardian’s presence during the appointment is necessary.
6. Present your printed application form at the Application Counter.
7. Proceed to the Information Counter to obtain your priority number.
8. Wait until your number is called.
9. Upon completing the necessary steps, you can proceed to the Delivery Counter. Here, you’ll decide whether to collect your passport at the DFA office or have it delivered to your residence. Regardless of your choice, your passport will be ready within 15 working days (regular) or 7 working days (expedited). The associated fees will vary based on your desired processing time.
10. With the process concluded, your only remaining task is to await the moment when you can claim your passport.

Supporting Documents

To complement your identification documents, the following supporting papers are required. Please make sure you have these ready:

– Old documents issued at least one year prior to the date of application that show the correct name, date, and place of birth, picture and signature of the applicant, such as the following:
– NSO Birth Certificate of child/children
– SSS-E1 Form or Microfilmed Copy of SSS-E1 Form
– Elementary or High School Form 137 or Transcript of Records with readable dry seal
– Voter’s ID
– Income Tax Return
– Land Title
– NBI Clearance
– NSO Marriage Contract
– Government Service Record
– Barangay Clearance
– Police Clearance
– School Yearbook
– Community Tax Certificate
– Voter’s Certification, List of Voters, and Voter’s Registration Record (for newly issued VRR, please attach receipt)
– Seaman’s Book

Acceptable Identification Documents (IDs)

When it comes to Valid IDs, these cards display your name alongside your photograph. While ideally, your PhilSys ID with one photocopy should suffice, if you haven’t obtained this ID yet, you can also furnish any of the following:

– Senior Citizen ID
– Social Security System (SSS) Card
– Government Service Insurance System (GSIS) Card
– Unified Multi-Purpose Identification (UMID) Card
– Philippine Postal ID (issued November 2016 onwards)
– Philippine Identification (PhilID)/ePhilID
– Land Transportation Office (LTO) Driver’s License. Student Permit may be accepted if in card format.
– Commission on Elections (COMELEC) Voter’s ID or Voter’s Certificate issued from COMELEC main office in Intramuros, Manila.
– Airman License (issued August 2016 onwards)
– Philippine National Police (PNP) Permit to Carry Firearms Outside Residence
– School ID (if applicable)
– Seafarer’s Record Book (SRB) or Seafarers Identity Document (SID) (*must be issued Feb 2020 onwards) issued by the Maritime Industry Authority (MARINA)
– Overseas Workers Welfare Administration (OWWA) E-Card
– Professional Regulatory Commission (PRC) ID
– Valid or Latest Passport (For Passport Renewal)
– Certificate of Enrolment with a photo of the minor and the school’s dry seal (*for minor applicants if school ID is not applicable)
– School ID and Certificate of Registration (*for adult applicants)
